During the process of professional consultation services, employment relationships, or service offerings related to Autism M.D. Clinic, we ensure that personal information is collected and processed lawfully and transparently. We commit to informing the individual about the purposes of collecting and processing their personal information, collecting it relevantly, and retaining it for the duration necessary for the purpose of this process, in accordance with the applicable retention rules. What types of personal information are we likely to collect? These may include, for example: - Identification data such as your name, RAM (Health Insurance Board) number, home address, phone number, email address, and those of your relatives when necessary; - Clinical data, related to the reason for consultation within our team such as your personal and family history, lifestyle habits, educational background, health status, reports from previous assessments and follow-ups, and any information relevant to the assessment and follow-up process that you have shared with us; - Data as a candidate, employee, or professional working within the Autism M.D. Clinic team including your name, address, email, phone, and all information you provide such as photo, cover letter, skills, education level, degrees, training and professional experiences, languages spoken, hobbies, family situation, as well as, if applicable, your employment contract, recruitment date, and financial data related to your employment or professional practice within our team. - Professional data such as name, title, your company affiliation, and contact and payment details if you are a supplier or service provider to Autism M.D. Clinic. Who can access your file? Members of the Autism M.D. Clinic team ensure the confidentiality and privacy of individuals who consult with us and those who work within our team. As part of the diagnostic process and the offered follow-up, we may share certain information among professionals working within our team. Only you, your legal representative, and authorized professionals may access your file. In compliance with the law, the professional who referred you for an evaluation will have access to the completed consultation report. Otherwise, we cannot transmit information from your file to a third party outside of Autism M.D. Clinic without your consent or that of your legal representative, except in exceptional situations such as serious and imminent danger, a court order, or in the context of a work or road accident, for example. The law also allows a healthcare professional to access records for research or educational purposes under certain conditions. As part of the provision of our services, some of our subcontractors may access your personal data. We select our collaborators carefully, ensuring that they provide sufficient guarantees, particularly in terms of expertise, reliability, and resources to meet the requirements of applicable legislation. For information purposes, here are the names of companies of our main external service providers: - ADP, payroll service (for Autism M.D. Clinic employees) - Logiforms Software Inc., online form generator - Info-Data Software "Ofys DMÉ", electronic medical record - Nethris, payroll service (for Autism M.D. Clinic employees) - SRFax, fax service - Xacte, billing service What is your role in respecting privacy? Unless specifically authorized, you are not allowed to capture/record images or videos related to your interaction with a professional from the clinic, whether your meetings are in person or through remote intervention via video or phone. You also have the responsibility to respect the confidentiality of other individuals receiving services at our clinic. Therefore, you cannot disclose information about a person other than yourself who is consulting at Autism M.D. Clinic unless you are their legal representative. Autism M.D. Clinic may update this Privacy Policy periodically. We encourage our patients to regularly review this policy. Who to contact if you have questions?
